Medical records and disability forms

It is important for patients to be aware of our administrative practices. Please review the following information about our administration fees and contact us if you have questions.

Medical Records
Medical records are available to patients at no charge for the first copy. Additional copies are $18.00 for the first page and $0.25/per page thereafter, not to exceed $22.50. Please contact our Medical Records Department at 541-686-3791 or fax requests to 541-686-3795. Learn how to request your medical records.

Disability Forms
There is a $15.00 administration fee (per document) for filling out short-term disability, FMLA, Oregon Paid Leave and all other disability related form(s); this fee is due when the form(s) are completed. Please submit these form(s) to our office via fax or hand delivery and allow 10 days after the fee is paid and the form(s) are submitted to your physician.
