It is important for patients to be aware of our administrative practices. Please review the following information about our administration fees and contact us if you have questions.
Medical Records
Medical records are available to patients at no charge for the first copy. Additional copies are $18.00 for the first page and $0.25/per page thereafter, not to exceed $22.50. Disability Forms
There is a $25.00 administration fee for filling out short-term or long-term disability form(s); this fee is due when the form(s) are completed. Please request these form(s) from our office, if needed, and allow 10 days after the fee is paid and the form(s) are submitted to your physician.
